Government-issued ID: PAN, Driving Licence, Aadhaar, etc.
Last 4 years' policy copies or policy numbers (individual holders)
Insurance agent name & phone number (individual holders)
HR name & phone number (corporate policy holders)
All documents within 12 hours of admission

The hospital does not approve or deny claims. All decisions are made by the insurance provider.
